The people that created policies that may be lacking, the lack of enforcement, the workers that refused to comply with potentially unsafe orders? Shouldn't companies have a policy where if an employee has concerns with an order for something legitimate, like safety, environmental impact [debatably], ethics, or something, they can talk to the person one level higher than the person they have the issue with without fear of loosing their income?
